For ADULT Renewal of ePassport:

1. Accomplished Passport Application Form (adult);

2. Personal Appearance;

3. Photocopy of Old Passport; and

4. Fee of ZAR 1,200

For Change Status - Single to Married 

PSA or NSO-issued Marriage Certificate (if married in the Philippines) or Report of Marriage issued by the Philippine Embassy will be further required

For MINOR Renewal of ePassport:

1. Accomplished Passport Application Form (minor);

2. Personal Appearance;

3. PSA or NSO issued Birth Certificate (if born in the Philippines), or Report of Birth issued by the Philippine Embassy (if born abroad); 

4. Photocopy of Old Passport;

5. Photocopy of Passport of either parent; and

6. Fee of ZAR 1,200

For New Passport (infant or 0-1 yr old) and Report of Birth

1. Accomplished Passport Application Form (minor);

2. Personal Appearance;

3. Accomplished Report of Birth Form;

4. Original and Photocopy of Birth Certificate (with English translation, if not in English);

5. Photocopy of Parent's Passports;

6. Original and Photocopy of Marriage Certificate of Parent; and

7. Fee of ZAR 1,200 for passport and/or ZAR 500 for Report of Birth

Additional requirements for Delayed Registration of Birth (more than 1 yr. old)

1. Affidavit of Delayed Registration of Birth and Fee of ZAR 500
