Requirements for Report of Birth:

The Report of Birth must be accomplished by either of the child’s parents or guardian. The child need not be present unless a new Philippine passport is applied for.

For Legitimate Child:

                   1. Four (4) Original Copies of accomplished Report of Birth Form,

          Reminder: In the date of birth of the child/parents and date of marriage of parents, the month should be spelled out, ex. 16 August 2017;

2. Original/Certified True Copy of Unabridged Birth Certificate of the child with four (4) photocopies;

3. Four (4) Photocopies of the passports of the child’s mother and father;

4. Original/Certified True Copy of the marriage certificate/contract of the child’s parents with four (4) photocopies;

5. For late Registration (One year and above), applicant should have to execute:

         Four Original Copies of the Affidavit of Delayed Registration of Birth, Fee of ZAR 500; and

6. Report of Birth Fee: ZAR 500.

For Illegitimate Child Acknowledged by Father:

                   1. Four (4) Original Copies accomplished Report of Birth Form,

         Reminder: In the date of birth of the child/parents and date of marriage of parents, the month should be spelled out, ex. 16 August 2017;

2. Four (4) Original Copies of Affidavit of Acknowledgement of Paternity and to Use Surname of Father executed by father; Fee: ZAR 500 

3. Four (4) Original Copies of Affidavit to Use the Surname of Father executed by mother, Fee: ZAR 500

4. Four (4) Original Copies of Certificate of Registration to be done by the Philippine Embassy; Fee: ZAR 500

5. Original/Certified True Copy of Unabridged Birth Certificate of the child;

6. Four (4) Photocopies of the passports of the child’s mother and father;

7. For late Registration (One year and above), applicant should have to execute

          Four Original Copies of the Affidavit of Delayed Registration of Birth, Fee of ZAR 500; and

8. Report of Birth Fee: ZAR 500.

For Illegitimate Child Not Acknowledged by Father:

                    1. Four (4) Original Copies of the accomplished Report of Birth Form,

         Reminder: In the date of birth of the child/parents and date of marriage of parents, the month should be spelled out, ex. 16 August 2017;

2. Original/Certified True Copy of Unabridged Birth Certificate of the child with four (4) photocopies;

3. Four (4) Photocopies of mother’s passport;

4. For late Registration (One year and above) applicant should have to execute

        Four Original Copies of the Affidavit of Delayed Registration of Birth, Fee of ZAR 500; and

5. Report of Birth Fee: ZAR 500.
