Requirements for Seaman / Seafarer Visa

• Personal appearance of the applicant
• Duly accomplished visa application form
• One (1) recent photograph, with white or light-colored background
• Applicant’s national passport valid for at least six (6) months or more from date of departure from the Philippines or completion of trip
• If the applicant is a seaman joining ship, the company concerned should provide a letter indicating the name of the vessel he will be joining in the Philippines
• Cash payment for application processing (ZAR 400)

Additional Requirements for Crew List Visa for Vessels

• Duly filled up Crew List Visa Form
• Photocopy of Vessel Master’s Passport
